Information you need if your loved one is going to apply for Social Security disability insurance.
If your loved one has become disabled due to an accident, injury, or illness, he or she needs to apply for Social Security disability insurance as soon as possible. Benefits do not start until the 6th full month of disability has passed, and the proper documentation needs to be in place in order for your loved one to collect any benefits.
To apply, your loved one will need original copies of:
- A work summary for the past 15 years
- Last year’s W-2, or if self-employed, last years federal tax return
- Dates of prior marriages
- His or her Social Security Number
- His or her birth certificate
- His or her military discharge papers if applicable
- His or her spouse’s birth certificate and Social Security number if he or she is applying for benefits
- Children’s birth certificates and Social Security numbers for children’s benefits
- Appropriate laboratory and test results
- Medical records from caseworkers, clinics, doctors, hospitals, and therapists
- Names, addresses, and phone numbers of doctors, hospitals, clinics, and institutions that treated your loved one and the dates treatment was received
- Names of all medications currently taken
- Proof of U.S. citizenship or lawful alien status if individual, spouse, or children were not born in the U.S.
- The bank name where your loved one would like his or her check direct deposited
